The purpose of this questionnaire is to ensure that the correct Control Station Combiner is chosen for each customer's application.
Please answer all questions:
1. How many Controls Stations are being combined?
2. What is the Control Station transmit power level?
3. What is the airline distance between the control station location and the repeater site location?
   (If the control stations are used to control multiple systems, list the distances to the closest and farthest systems)

4. Is the path from the control station antenna location to the repeater site a line of sight path or blocked? 
If the path is blocked, a measurement of the received repeater signal at the CSC antenna location is required to
determine the effective path loss.
